RULES FOR

SHOOT OUT FOR $100,000 HOLE IN ONE

WITNESSES & VIDEO RECORDING: All official third parties of a hole-in-one must be non-participants and age 18 or older and appointed by sponsor or responsible tournament official as follows: Prize values of $60,000 to $150,000 must be witnessed by at least two (2) of the other registered participants playing in the group of the registered participant making the attempt and two (2) official third party witnesses, one of whom is positioned at the tee box and one of whom is positioned at the green of the designated hole. The official third party witnesses must view each registered participant’s initial shot as well as the full and complete path of the ball until it comes to a stop. A video is also required, in addition to the witness requirements, for all agreements in which both: the number of contestants is 24 or less AND the Prize Value is $25,000 or greater. The video must provide a continuous, unedited video record of each registered participant’s initial shot from behind the green clearly showing the participant in the distance and the ball landing on the surface and coming to rest within the hole. All liability for the safety of the witnesses is borne by the association member and not USHIO.

YARDAGE: The target hole will measure from tee to flagstick, no less than the specified yardage in the certificate. Women may shoot at the target hole at a distance of up to fifteen (15) yards less than the men.

SHOTS: A “shot” is a stroke as defined by the USGA Rules of Golf. No practice shots or mulligans are permitted on the target hole. Each category of authorized number of shots is permitted only during the stipulated round of the tournament on the date specified in the certificate.

ELIGIBILITY: The hole in one must occur by an officially registered player in the specific tournament listed in the certificate. If any past or present professional golfer is eligible to win any hole in one prize, that individual must be counted as a professional player in the contest.

PUTT FOR $10,000

WITNESSES & VIDEO RECORDING: All official third parties of a putting contest must be non-participants and age 18 or older and appointed by sponsor or responsible tournament official as follows: Prize values up to $10,000 must be witnessed by at least one official third party. All putting events must be videotaped. For the Single Putt Contest, the video recording must be continuous, unedited and include the following: measurement of the putt distance, ball placement and the ball being struck and ending up in the bottom of the hole.

DISTANCE: The distance of the putt must measure no less than the distance specified on the Certificate of Insurance. In the event that the number of contestants taking a putt specified on the Certificate of Insurance is greater than one (1), for putts at the same target hole and distance, putts must be stroked from no closer than 15 feet along the circumference, so that subsequent putters cannot “Go to School” on previous putters.

NUMBER OF AMATEURS/PUTTS: The number of Amateurs taking a putt at the prize must be the same as the number of contestants taking a putt specified on the Certificate of Insurance. A “putt” is defined as a stroke by the USGA Rules of Golf taken by an officially pre-registered contestant. Only one putt per contestant/finalist from each distance is permitted. No practice shots or mulligans, or substitute shots for other players are permitted. No practice putts or elimination rounds are allowed at the target hole prior to the prize putt(s). If an elimination round is held to determine the contest(s), the elimination round putts cannot be held at the target hole and cannot exceed 50% of the length of the prize putt in length (ex. If the prize putt is 60 feet, the elimination round putts cannot be longer than 30 feet in length).

TARGET HOLE: A regulation (as defined by the USGA Rules of Golf) four and one quarter (4 ¼) inch hole must be used. The hole location is not permitted in a “valley” or concave portion of a green. No side rails or grooves are permitted which would enhance the probability of a putt being made. No claim can be made under this agreement, unless the putt achieved during the contest on the date, hole location, distance, and prize value specified in the Certificate.
